Forms & Policies
Hours of Operation:
Mon-Thurs 11am - 7pm
Correspondence:
Your provider will respond to emails, calls, and texts during business hours, and within approximately one business day.
Unless there is an emergency, correspondences will not be returned on weekends.
Emergencies:
Please be aware, we are unable to provide crisis, urgent, or emergency services.
If you are in need of emergency services, call 911 or proceed to the nearest hospital emergency room for immediate assistance. If you have an urgent problem after hours which cannot wait until the next business day, you can email our office at alice@transcendentalwellness.com and our staff and/or practitioner will get back to you as soon as available.
You may also call the MA Emergency Services Programs statewide number (877) 382-1609 for your local crisis team, which is available at any time.
Refills:
Medication refill requests can only be completed during business hours.
Requests on weekends will be filled the following non-holiday business day.
In order to change medications or adjust dosages, you must be seen for an appointment.
Refills will be provided to you at your visit to ensure you will have enough medication to make it to your next scheduled appointment. In the event there is an emergency or a cancellation of your appointment, refills will be provided until your next appointment.
Medication Compliance:
Patients have the responsibility to follow their medication plan. If you have any confusion regarding how you are supposed to take your medication, or you believe you may have been taking your medication incorrectly, please reach out to us.
If there is any reason you are unable to follow your medication plan, it is required that you share this with your provider.
Intentionally declining to adhere to your treatment plan will result in discharge from services.
Appointments and Cancellation Policy:
Please arrive on time for your appointment. If you are aware that you are going to be late, please call and let us know as soon as you are able.
If you are more than 10-minutes late to a 30-minute appointment, or more than 20-minutes late to an hour-long appointment (or eval), the appointment will be cancelled, and the missed appointment fee will apply.
24-hour notice is required to cancel an appointment to avoid the missed appointment fee of $100, except in the case of accidents, emergencies, or sickness, which will not be charged.
Should you miss two consecutive appointments or if we are unable to contact you within 2 weeks of a missed appointment to reschedule, we will assume that you no longer wish to receive services with us, and we may close your treatment file.
Also, we reserve the right to terminate services if you fail to cancel appointments on a recurring basis without providing 24-hour notice.
